How To Get Your Records Of Immunization In Ontario
Immunization Connect Ontario is a web-based service provided by the Ministry of Health and Long-Term Care that allows you to submit immunization information. You can also access your immunization record using a unique Ontario Immunization Identification (OIID) and a Personal Identification Number (PIN) .
